To obtain an insurance payment on the death of the insured person the following documents must be presented to the JSC "SOGAZ":
1) An application of each beneficiary of the insured for an insurance payment. If the beneficiary of the insured person is a minor or incompetent person, an application is issued by his legal representative attaching the documents confirming the right to act in the interests of the minor or incompetent beneficiary of the insured (Application);
2) A notarized copy of death certificate;
3) A notarized copy of passport of the applicant (for minor or incompetent beneficiaries – a notarized copy of birth certificate attaching a notarized copy of passport of his legal representative);
4) The original or a notarized copy of a document issued by the competent authority of the State of nationality of the insured, specifying the list of the beneficiaries and their share of inherited property;
5) The bank account for non-cash transfer of funds;
6) Documents issued in foreign languages should be accompanied by a Russian translation (apostil).
Upon review of the documents OJSC "SOGAZ" may request the additional documents necessary for insurance payments.
